Upon purchasing this program, I hereby acknowledge that I am voluntarily participating in an exercise program offered by Dr. Samantha Smith PT. I understand that the program may involve strenuous physical activity, and I am fully aware of the risks and hazards involved.

I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the program. I represent and warrant that I am in good physical condition and have no medical condition that would prevent my participation in the program.

In consideration of being allowed to participate in the personal training program, I do hereby waive, release, and forever discharge Dr. Samantha Smith PT.

Succeed Courses and their respective officers, agents, employees, and representatives from any and all claims, demands, damages, rights of action, or causes of action, present or future, arising out of or connected with my participation in this or any exercise program, including any injuries resulting theretrom.

This waiver and release of liability includes, without limitation, injuries which may occur as a result of (a) equipment that may malfunction or break, (b) any slip, fall, or dropping of equipment, (c) and/or improper maintenance of any equipment, (d) negligent instruction or supervision, or (e) any other cause, whether foreseeable or unforeseeable by the parties hereto.

I acknowledge that I have carefully read this waiver and release of liability and fully understand its contents. I voluntarily and knowingly agree to the terms and conditions stated herein.
